Abstract

Abstract: Type 2 Diabetes Mellitus is a chronic metabolic disease that often leads to complications, one of which is unstable peripheral perfusion due to reduced blood flow to the extremities. This condition can cause diabetic ulcers and even lead to amputation if not treated properly. One of the preventive interventions that can be done is foot care for diabetic patients. This case study aims to describe nursing care for a patient with unstable peripheral perfusion through foot care interventions in Aqsa 2 Room at RSUDZA Banda Aceh. The study used a case study method with a nursing process approach that includes assessment, nursing diagnosis, planning, implementation, and evaluation. The patient came with complaints of dizziness and paresthesia, with a blood glucose level of 260 mg/dL and a history of type 2 diabetes for 10 years. On the first day of evaluation, the patient’s GDS was 275 mg/dL, CRT was more than 3 seconds, peripheral pulse was weak, extremities were cold, lower leg skin appeared pale, hemoglobin was 11 g/dL, and skin elasticity was reduced. The unstable peripheral perfusion was mainly due to the lack of proper foot care. After seven days of foot care interventions like diabetic foot exercise, there was a decrease in GDS to 200 mg/dL, less paresthesia, improved peripheral pulse, and better skin elasticity.
